Blank two-sided card form (see both sides in photo). "(A.G.O., NO. 9, JUNE 3, 1898, Vols.)" "Note- One of these cards will be prepared in the cases of individual enlistments. When recruits are sent from a rendezvous or station, the cards will be given to the officer assigned to the command of the detachment, or if no officer be so assigned, the cards will be sent by mail. Upon the arrival of a recruit at his detination, the regimental or other commander will furnish the company commander with a copy of the card and forward the original direct to the Adjutant General of the Army. Should the recruit fail to report within a reasonable period, the fact will be noted on the card and the latter be forwarded to the Adjutant General of the Army."
