Signed by McCaleb and Paymaster L.J. Thurston. Note of addition at bottom "and am not entitled to compensation for food and quarters any of time charged." McCaleb ended up committing suicide after the war due to pain from his war injuries. Archival document repair tape to seams along folds where paper has separated.
